Used for cadets on academic probation to participate in JROTC events and activities. |

| Flag Merit/Demerit Form |
Used by cadets to recommend merits for cadets assisting with the school flag, and demerits for those who are absent. |
| Merit Application |
Used by cadets at the end of each grading period to get credit (merits) for extra-curricular activities, events, and achievements. |
| Merit/Demerit Form |
Used by cadets to submit recommendations, to the instructors, for merits (rewards) or demerits (disciplinary action). |
| Parent Permission |
Used for events after school hours that are away from school property. |

Parent/Teacher Pemission
|
Used for cadets participating in events that cause a student to miss a class, or leave school early.
